Discharge Procedures

Early during your hospitalization, your health care team will talk to you about your after-hospitalization care needs, your anticipated discharge date, and how to receive your follow-up care.  This process includes a social worker and a patient care coordinator who will continue to follow your case throughout your hospitalization.  They, in conjunction with your physicians and nurses, are able to make referrals for assistance at home, are able to help obtain supplies or equipment required at home, and are able to help to assist with transportation home from the hospital when necessary. 


If medication is prescribed at the time of discharge, it is delivered to your room by the discharge pharmacist.  The discharge pharmacist and your physician will go over any changes that were made in your medications during the hospitalization.


A dietician will follow you throughout your hospitalization and provide counseling to you and/or your family on how to follow your diet at home before discharge.


Our hospital also has a discharge lounge available for patients who are waiting for transportation.
